Information for Referring Doctors

Thank you for considering Nephrology West for your patient's kidney care. We aim to provide timely, high-quality specialist consultations with clear communication back to you.

How to Refer

Referrals can be sent via:

​

Wait Times

Routine: Approximately 6 weeks

Urgent: Within 1-2 weeks


All referrals are reviewed within 2-3 business days. We will contact the patient if clinical information suggests earlier review is needed.

Criteria for Urgent Referral

Please indicate URGENT on your referral if the patient has:

  • Rapidly declining eGFR (>5 mL/min/1.73m² drop in 3 months)

  • eGFR <15 mL/min/1.73m² (unless established on conservative pathway)

  • Significant hyperkalaemia (K >6.0 mmol/L) not responding to initial management

  • Nephrotic syndrome (uACR >300 mg/mmol with hypoalbuminaemia and oedema)

  • Active urinary sediment (dysmorphic RBCs, RBC casts) suggesting glomerulonephritis

  • Suspected rapidly progressive glomerulonephritis or vasculitis

  • Accelerated/malignant hypertension with renal impairment​

Helpful Information to Include in Referrals

  • Reason for referral and any specific questions

  • Recent lab results (UEC, uACR/uPCR, FBC)

  • Current medication list

  • Relevant imaging report (renal ultrasound or CT)

  • Relevant comorbidities (diabetes, hypertension, cardiac history)

Areas of Special Interest

Dr Sri has particular expertise in:

  • Autoimmune kidney disease (vasculitis, lupus nephritis, glomerulonephritis)

  • Resistant/secondary hypertension

  • Conservative (non-dialysis) kidney care for advanced CKD

Communication

You will receive a consultation letter following each patient visit, sent via your preferred method (Healthlink, email, fax, or post). If you have not received correspondence within 2 weeks of your patient's appointment, please do contact us.
